📋 Description

• Schedule and perform in-person home assessments.
• Develop relationships with Case Management Agencies for FreedomCare.
• Serve as field expert for waiver member and caregiver onboarding.
• Partner with Care team to monitor hospitalizations and needs.
• Maintain regular contact with caregivers and waiver members.
• Represent FreedomCare at events and with care teams.

🎯 Requirements

• Bachelor’s degree or equivalent experience.
• 2+ years serving frail or elderly disabled adults and caregivers.
• 1-3 years medical field experience and/or case management.
• Strong verbal and written communication.
• Excellent time management and multitasking.
• HIPAA compliance and patient confidentiality.
