📋 Description

• Build trusting, ongoing relationships with patients, families, caregivers, providers, and community
• Engage high-risk populations via bi-directional communication to address health questions and
• Identify medical, behavioral health, social, and financial needs to support whole-person care.
• Help patients understand and follow care plans, health goals, and follow-up.
• Provide patient education and motivational interviewing support within scope to encourage
• Escalate changes in patient condition or barriers using structured workflows.

🎯 Requirements

• At least 3 years in patient navigation, care coordination, peer support, or similar roles.
• Experience with EHR systems or similar platforms.
• Remote work experience and ability to work independently.
• Knowledge of medical terminology, primary care workflows, and social drivers of health.
• Strong communication, problem-solving, and organizational skills.
• Ability to collaborate with patients, families, and care teams.
