Home Care Coordinator

📋 Description

• Coordinate home care services to deliver person-centered care at home.
• Participate in assessment of participant needs and care plan development.
• Collaborate with the Interdisciplinary Team (IDT) to align services.
• Demonstrate Habitat's values in daily care.
• Conduct in-home assessments, safety reviews, and service appropriateness.
• Coordinate with participants, caregivers, and families on care gaps.

🎯 Requirements

• Active LVN license in the applicable state.
• Minimum 2 years clinical experience in home care or geriatrics.
• Knowledge of care planning, clinical assessments, and service delivery.
• Excellent interpersonal, communication, and collaboration skills.
• Ability to work within an Interdisciplinary Team (IDT).
• Proficiency with electronic health records (EHR) and documentation systems.
