Home Care Coordinator

📋 Description

• Conduct in-home care assessments and set framework for the home care team.
• Arrange, assess, and oversee personal care in participants' homes.
• Coordinate calls with participants, physicians, and agency services.
• Communicate schedule changes with nursing, therapy, aides, and physicians.
• Collaborate with Home Care Services to track delivery and hours.
• Partner with Marketing to enroll prospective participants.

🎯 Requirements

• Healthcare/medical licensure or equivalency with 3+ years of experience.
• Bachelor's degree preferred.
• Minimum 3 years of case management or nursing in clinical/home care with frail elderly.
• Nursing knowledge to treat frail elderly and complex conditions preferred.
