Home Care Coordinator

📋 Description

• In-home care assessments and care planning
• Coordinate, arrange, and oversee personal care in participants\' homes
• Collaborate with IDT and home care team to ensure participant needs are met
• Support enrollment and onboarding of participants into the program
• Coordinate calls with participants, physicians, and agency services
• Monitor scheduling hours and delivery of home care services

🎯 Requirements

• Healthcare/medical licensure or equivalent; at least 3 years of professional experience
• Bachelor\'s degree preferred
• Minimum 3 years of case management or nursing experience with frail/elderly population
• Nursing knowledge for frail elderly participants and complex conditions preferred
