Home Care Coordinator

📋 Description

• Coordinate and oversee in-home care assessments for participants
• Structure and schedule home care assessments around your desired hours
• Collaborate with IDT, nursing, therapy, aides, social services, and physicians
• Manage participant calls, referrals, and scheduling changes by phone
• Support enrollment and onboarding of prospective participants with Marketing
• Assist with staffing, scheduling, and end-of-life care coordination

🎯 Requirements

• Healthcare/medical licensure or equivalent with at least 3 years of professional experience
• Bachelor’s degree preferred
• Minimum 3 years of case management or nursing in clinical or home settings with frail/elderly
• Nursing knowledge and training for frail elderly participants is preferred
