Home Care Coordinator

📋 Description

• Coordinate incoming calls related to participants, physicians, and agency services regarding
• Communicate with participants by phone and coordinate effectively with nursing, therapy, aides
• Collaborate with Home Care Services staff to track and monitor home care delivery and scheduling
• Partner with the Marketing team to support enrollment of prospective participants
• Assist with staffing and scheduling activities
• Participate in end-of-life care, coordination, and support

🎯 Requirements

• Healthcare/medical licensure or equivalency, along with at least three years of professional
• Bachelor's degree preferred
• Minimum of three years of case management or nursing experience in a clinical or home setting with
• Nursing knowledge and training to treat frail, elderly participants and care for complicated
