Review Participant Care Plans to coach HCCs on documentation for compliance
Oversee pre-enrollment assessments; coordinate with MOE team; attend weekly meetings
Manage direct reports including hiring, training, supervising and mentoring
Lead investigations between HCAs and participants; coordinate with HR
Lead daily meetings with HCCs to discuss pending assessments and hospitalizations
Check-in with HCCs to review cases and coach on IDT discussions

🎯 Requirements

Healthcare or medical licensure plus 3 years of professional experience
Bachelor’s degree preferred
Minimum 3 years of case management or home nursing with frail/elderly
CPR and First Aid certification within 180 days; certification on hire preferred
