Direct a high-performing home care dept, ensuring workflows and policies are followed
Guide and mentor home care staff, including hiring, training, and coaching
Assess effectiveness of home health nursing assessments and IDT participation
Ensure nurses deliver and document interventions per care plans
Oversee care quality in nursing homes and board and care facilities

🎯 Requirements

Bachelor's degree preferred; additional relevant experience may substitute
Min 2 years nursing in home health with frail/elderly population preferred
At least 1 year leadership experience in Home Health (preferred)
California RN license required
Knowledge of nursing for frail elderly patients and complex cases
