Home Health Nurse- LPN/ RN

📋 Description

• Provide continuous, high-acuity 1:1 skilled nursing care tailored to the client's complex medical
• Manage advanced clinical care needs, which may include tracheostomy, ventilator support, feeding
• Perform thorough physical assessments, monitor daily vital signs, and track changes in condition.
• Respond promptly and effectively to subtle changes in health status or clinical emergencies.
• Assist with activities of daily living (ADLs) and promote patient comfort, dignity, and mobility.
• Maintain accurate, detailed, and timely electronic charting and documentation.

🎯 Requirements

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills
