Home Health RN

📋 Description

• Conduct home health nursing assessments to determine equipment needs and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions per care plans, meds, tests.
• Instruct participants and families on disease process and self-care.
• Track home health scheduling with the Home Health Manager.
• Collaborate with physicians and IDT for smooth care transitions.

🎯 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN license.
• Minimum 1 year nursing experience with frail/elderly; home health experience preferred.
• Reliable transportation.
• CPR and First Aid within 180 days; maintain certification thereafter.
• CPR/First Aid certification upon hire preferred.
