Home Health RN

📋 Description

• Provide skilled home health care per CMS guidelines.
• Coordinate care with the interdisciplinary team (IDT).
• Conduct nursing assessments and equipment planning in the home.
• Administer meds, perform tests, and follow physician orders.
• Educate patients and families on disease, self-care, and prevention.
• Track scheduling with the Home Health Manager/IDT for care flow.

🎯 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered RN license required.
• Minimum one year nursing experience with frail/elderly; HH experience preferred.
• Reliable transportation.
• CPR and First Aid certification within 180 days of hire; maintain thereafter.
