Conduct home health nursing assessments to determine equipment needs and goals.
Actively participate in IDT meetings to develop patient care plans.
Deliver and document nursing interventions per care plans, including meds and tests.
Instruct participants, families, and caregivers on disease processes and self-care techniques.
Track and monitor scheduling with the Home Health Manager.
Collaborate with physicians and IDT for smooth care transitions.

🎯 Requirements

Graduate of an accredited school of nursing.
Unencumbered RN license required.
Minimum of one year nursing experience with frail/elderly; home health preferred.
Reliable means of transportation.
CPR and First Aid certification within 180 days of hire; preferred.
