Home Health RN

📋 Description

• Conduct home health nursing assessments for equipment needs and goals.
• Actively participate in IDT meetings to develop care plans.
• Deliver nursing interventions per care plans and administer meds as ordered.
• Instruct patients and families on disease process and self-care.
• Track and monitor home health scheduling with the Home Health Manager.
• Collaborate with physicians and IDT for smooth care transitions.

🎯 Requirements

• Graduate of an accredited nursing school.
• Unencumbered RN license required.
• At least 1 year nursing experience with frail/elderly; home health preferred.
• Reliable means of transportation.
• CPR and First Aid certification within 180 days of hire (preferred).
