📋 Description

• Make home visits to clients in designated territories.
• Conduct evaluations to diagnose and treat speech, language, swallowing issues.
• Develop treatment plans per evaluation, physician orders, and goals.
• Update care plans and collaborate with the multidisciplinary team.
• Educate patients and families on rehab, safety, and independence.
• Document observations and interventions using a touch pad EMR tablet.

🎯 Requirements

• MD license as Speech-Language Pathologist.
• ASHA CCC-SLP or eligible to take certification exam.
• Education and supervised experience required for certification.
• Solid computer skills; EMR experience preferred.
