📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and diagnose, provide interventions for speech, language
• Develop treatment plans based on evaluation, physician orders and goals.
• Revise the care plan and participate in multidisciplinary team discussions as appropriate.
• Educate patients and families in rehabilitative care to promote health and independence.
• Document observations and interventions using a touch pad device.

🎯 Requirements

• Current Rhode Island Speech-Language Pathologist license.
• One year of experience as an SLP (preferred but not required).
