📋 Description

• Provide nursing and rehabilitative care for geriatric patients in Assisted Living and Independent
• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Manage acute and post-acute care needs (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and independently manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Complete wound assessment, care, and documentation.

🎯 Requirements

• A current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited nursing program with a transcript or diploma.
• At least one year of hands-on medical experience with adult/geriatric population in relevant
