• Minimum 2 years with physicians, hospitals, insurers, or healthcare partners.
• Solid understanding of hospice philosophy, eligibility criteria, and interdisciplinary care.
• Ability to work independently and collaboratively in a multidisciplinary team.
• Valid driver's license and reliable transportation.
