Hospice Case Manager-RN, Full time

📋 Description

• Making home visits to clients in designated geographic territories.
• Administering medications, wound care, treatments, and procedures; provide comfort care.
• Monitor clients' conditions and report changes to Clinical or Client Services Manager.
• Follow up on, execute, and properly document doctors' orders.
• Perform client assessments as needed.
• Occasional on-call after hours and weekends.

🎯 Requirements

• Current RN license in VT and NH.
• Minimum of one year of recent verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience preferred, but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
