Hospice Case Manager-RN, Full time

📋 Description

• Making home visits to clients in designated territories.
• Administer meds, wound care, treatments, and comfort care.
• Monitor client conditions and report changes.
• Follow up, execute, and document doctors' orders.
• Perform client assessments as needed.
• Occasional on-call after hours and weekends.

🎯 Requirements

• Current RN license in VT and NH.
• Minimum one year of recent clinical (medical/surgical) experience.
• Home care and/or hospice experience preferred, not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
