Hospice RN Case Manager

📋 Description

• Complete comprehensive nursing assessments evaluating physical, emotional, and psychosocial needs.
• Develop and manage individualized care plans with patients, families, physicians, and the team.
• Monitor patient progress and adjust care plans to maximize comfort and quality of life.
• Serve as the primary clinical resource for patients and families throughout their hospice journey.
• Assess and manage pain and end-of-life symptoms; educate on medications and progression.
• Coordinate care with nursing, social work, chaplaincy, aides, and physicians; on-call.

🎯 Requirements

• Current, unrestricted RN license in the state of practice.
• Minimum one year of nursing experience.
• Valid driver's license and ability to travel throughout service area.
• Hospice, palliative care, home health, or case management experience preferred.
• Strong clinical assessment and care planning skills.
• Experience educating patients and caregivers; proficient in EHR/EMR; excellent communication.
