📋 Description

• Enroll patients and caregivers into FreedomCare Colorado programs.
• Liaise among consumers, providers, and Case Management Agencies.
• Manage onboarding from qualification to On Care.
• Educate patients and caregivers on application and enrollment steps.
• Collaborate with medical providers to ensure documentation compliance.
• Meet monthly metrics for starting care and call handling.

🎯 Requirements

• Experience handling high-volume inbound/outbound calls.
• Exceptional customer service with empathy, active listening, and patience.
• Salesforce proficiency, including Chatter and detailed notes.
• Ability to document statuses clearly in Salesforce and manage case notes.
• Proficient with computers, fast typing, and navigating multiple portals.
• Bilingual in Spanish, or other languages desired.
