📋 Description

• Serve as the primary point of contact and trusted advisor for patients and caregivers throughout
• Review and assess eligibility and program-fit requirements (Medicaid, insurance, MCO, AAA/CMA
• Manage and maintain accurate Service Record and Enrollment Record documentation in Salesforce
• Coordinate and track all required enrollment and caregiver onboarding items
• Facilitate communication with health plans, waiver programs, case managers, state agencies, and
• Schedule, coordinate, and reschedule intake visits, orientations, assessments, and callbacks

🎯 Requirements

• High school diploma, bachelor’s degree preferred
• Minimum of 3 years of experience in sales, community outreach, or related roles
• Strong customer service and communication skills
• Strong typing and computer proficiency
• Effective problem-solving and critical-thinking skills
• Ability to follow scripts, workflows, and regulatory requirements
