📋 Description

• Making home visits to clients in designated territories.
• Administer medication, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to managers.
• Follow up, execute, and document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination; document care using EMR on a tablet.

🎯 Requirements

• A current Licensed Practical Nurse license in VA.
• Minimum one year of LPN experience.
• Graduated from an accredited nursing program.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time.
• Strong interpersonal and EMR skills.
