📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to Clinical and Client Services Managers.
• Execute and document doctors' orders; follow up as needed.
• Perform client assessments and care management.
• Document observations using EMR on a state-of-the-art tablet.

🎯 Requirements

• A current VA LPN license.
• Minimum one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program (transcript/diploma).
• Prior home care experience strongly preferred.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
