📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, treatments as ordered.
• Monitoring clients' conditions and reporting changes.
• Following up on and documenting doctors' orders.
• Performing client assessments as needed.
• Case management and coordination using EMR on a touchpad tablet.

🎯 Requirements

• A current NJ Licensed Practical Nurse license.
• Minimum of one year of recent LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and basic EMR/computer skills.
