Making home visits to clients in designated territories.
Administer medication, wound care, treatments, and procedures.
Monitor clients' conditions; report changes to Clinical Manager and Client Services Manager.
Follow up with, execute, and document doctors' orders.
Perform client assessments as necessary.
Case management and coordination; document care in EMR on a touch pad tablet.

🎯 Requirements

A current DE LPN license.
Minimum one year of recent LPN experience.
Graduation from an accredited nursing program.
Ability to work independently and manage time.
Strong interpersonal skills.
Solid computer skills; EMR experience preferred.
