Licensed Practical Nurse- Home Health

📋 Description

• Making home visits to clients in designated territories.
• Administer medication, wound care, treatments, and procedures.
• Monitor client conditions; report changes to Clinical Manager and Client Services Manager.
• Follow up on, execute, and document doctors' orders.
• Perform client assessments as needed.
• Case management and coordination with EMR documentation.

🎯 Requirements

• A current DE LPN license.
• Minimum of one year of recent LPN experience.
• Graduation from accredited nursing program (transcript/diploma).
• Prior home care experience preferred, but not required.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
