📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, treatments, and procedures.
• Monitoring client conditions and reporting changes to Clinical Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination of care; documenting care in EMR.

🎯 Requirements

• Active DE LPN license.
• At least 1 year of LPN experience.
• Diploma/transcript from accredited nursing program.
• Prior home care experience preferred.
• Able to work independently, manage time; travel as needed.
• Strong interpersonal skills; proficient with EMR.
