📋 Description

• Making home visits to clients in designated territories.
• Administer medication, wound care, treatments and procedures.
• Monitor client conditions; report changes to Clinical Manager.
• Follow up with and document doctors' orders.
• Perform client assessments as needed; case management and coordination.
• Accurately document observations and interventions in EMR on a touch-pad tablet.

🎯 Requirements

• Current DE LPN license.
• Minimum 1 year LPN experience.
• Graduate from accredited nursing program.
• Home care experience preferred.
• Ability to work independently and manage time.
• Strong interpersonal skills.
• Proficient computer skills; EMR experience preferred.
• Ability to travel to cases.
