Making home visits to clients in designated territories.
Administer medications, wound care, treatments, and procedures.
Monitor client conditions and report changes to Clinical/Client Services Managers.
Follow doctors' orders and document them accurately.
Perform client assessments and coordinate case management.

🎯 Requirements

Current Pennsylvania LPN license.
Minimum 1 year recent LPN experience.
Graduation from an accredited nursing program.
Prior home care experience preferred.
Ability to travel to assigned cases.
Solid computer skills; EMR experience preferred.
