Licensed Practical Nurse (LPN) - Home Health

📋 Description

• Making home visits to clients in designated territories.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and documenting doctors' orders.
• Case management and coordination.
• Document observations and evaluations using EMR on a touch pad tablet.

🎯 Requirements

• Current North Carolina LPN license.
• Minimum 1 year LPN experience.
• Graduation from accredited nursing program.
• Home care experience preferred, not required.
• Ability to work independently and manage time effectively; strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
