Licensed Practical Nurse (LPN) - Home Health

📋 Description

• Make home visits to clients in designated territories
• Administer medications, wound care, treatments, and procedures
• Monitor client conditions and report changes to Clinical Manager
• Follow up with, execute, and document doctors' orders
• Perform client assessments as necessary
• Case management and coordination; document care in EMR on tablet

🎯 Requirements

• One year of LPN clinical experience required
• Current North Carolina LPN license
• Graduate from an accredited nursing program
• Home care experience preferred but not required
• Ability to work independently and manage time
• Strong interpersonal skills and EMR proficiency
