Licensed Practical Nurse, LPN, Home Health Visits

📋 Description

• Making home visits to clients in designated Erie County territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical Manager.
• Follow doctors' orders and document actions in EMR.
• Conduct client assessments and coordinate care plans.
• Travel to cases as assigned.

🎯 Requirements

• PA license as a Licensed Practical Nurse (LPN).
• Minimum of 1 year LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred.
• Ability to work independently and manage time.
• Strong interpersonal skills.
