Licensed Practical Nurse (LPN)- Home Health Visits

📋 Description

• Provide skilled nursing and rehabilitative care to adult clients in their homes on a per-visit
• Make home visits to clients in designated territories in Dauphin and Cumberland counties.
• Administer medications, wound care, treatments, and procedures per physician orders.
• Monitor client conditions and report changes to Clinical Manager and Client Services Manager.
• Document observations and interventions in electronic medical records.
• Collaborate with a multidisciplinary health care team to support home-based care.

🎯 Requirements

• One year of clinical experience as a Licensed Practical Nurse (LPN) required.
• Current LPN license in Pennsylvania.
• Graduate from an accredited nursing program; strong EMR skills preferred.
• Ability to travel to cases as assigned; independent time management; strong interpersonal skills.
• Prior home care experience preferred but not required.
