Licensed Practical Nurse, LPN, Home Health Visits

📋 Description

• Make home visits to clients in designated territories in Cuyahoga County.
• Perform duties including medication administration, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical/Client Services Managers.
• Follow up on doctors’ orders and document them properly.
• Conduct client assessments as needed and assist with case management and coordination.
• Document observations and care using electronic medical records on a state-of-the-art touch pad

🎯 Requirements

• Current OH Licensed Practical Nurse license.
• Minimum of one year LPN experience.
• Graduate from accredited nursing program.
• Home care experience preferred; not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and solid computer/EMR skills.
