Licensed Practical Nurse (LPN) - No Weekends

📋 Description

• Travel between Kokomo, Peru, and Lafayette areas to cover care.
• Provide LPN care and health management to residents with disabilities.
• Conduct site visits, check vitals, assessments, med administration, document progress.
• Train staff on medical topics; act as health resource and med audits.
• Ensure compliance with state requirements and medical records policies.
• Full-time Mon-Fri; occasional travel; on-call triage 1 week per month.

🎯 Requirements

• Indiana LPN license required.
• 1–2 years with disabilities or long-term care preferred.
• Skilled, flexible, self-motivated, dependable; team player.
• Strong verbal and written communication skills.
• Ability to work independently and efficiently.
• Organizational and computer skills; background and driving record checks.
