Licensed Practical Nurse (LPN) - No Weekends

📋 Description

• Provide quality, individualized health care management for individuals with disabilities
• Conduct routine site visits, take vitals, assess, counsel meds, document progress
• Act as medical resource, train staff in medical arena
• Conduct medication audits and ensure regulatory compliance
• Schedule: full-time Mon-Fri, some travel, on-call triage 1 week/mo

🎯 Requirements

• Currently licensed Practical Nurse (LPN) in Indiana
• 1-2 years in disability/long-term care preferred
• Strong verbal/written communication, independent work, team player
• Organizational and computer skills; acceptable background/driving checks
