Make home visits to clients in designated geographic territories.
Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
Identify appropriate community resource referrals to address practical and environmental needs.
Advocate for client and family to navigate the patient through the community system.
Participate in development of the total plan of care with BAYADA clinicians.
Develop and maintain contracts with public and private agencies as resources for the patient.

🎯 Requirements

A Master’s Degree in Social Work
Licensed Social Worker (LCSW or LICSW) in the State of Colorado
At least one year of experience working independently as a Licensed Social Worker

Base Pay: $65 - $75 per point, depending on qualifications
