Licensed Social Worker (Part-Time / Field-based)

📋 Description

• Interpret the agency's philosophy and objectives to clients and families.
• Serve as escalation point and advocate for care quality.
• Conduct initial in-person home assessments for the plan of care.
• Provide day-to-day monitoring with monthly contact with the patient and caregiver.
• Ensure program integrity; triage issues to internal staff as needed.
• Support independence and home-based care within the SFC program.

🎯 Requirements

• Valid Ohio LSW license (CSWMFT Board).
• BSW or MSW in Social Work from CSWE-accredited program.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities.
• Home care experience a plus.
• Bilingual in Spanish is a plus.
• CRM experience; Salesforce is a plus.
• Valid driver's license with an insured vehicle for travel within the state.
