Licensed Vocational Nurse

📋 Description

• Provide direct nursing care to PACE participants as directed by RN/physician
• Administer meds/injections; monitor vitals; observe health status
• Perform triage and prioritize care using standardized protocols
• Coordinate care transitions between home, day center, and clinic
• Implement care plan interventions and document changes
• Maintain accurate EHR/documentation and comply with policies

🎯 Requirements

• Current California LVN license in good standing
• 1+ year nursing experience; geriatrics/home health preferred
• CPR/BLS certification
• Experience with EMR/EHR systems
• Strong communication and teamwork with RN/physician
• Bilingual skills (preferred)
