📋 Description

• Ensure that each individual receives quality, individualized health care management, resulting in
• Conduct routine site visits, check vitals, do assessments, counsel medication administration, and
• Act as a health/medical resource for staff, training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure compliance with state requirements and organizational policies regarding medical care and
• Participate in on-call rotation for triage of medical concerns one week per month

🎯 Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Skilled, flexible, self-motivated, dependable, able to work cooperatively with staff
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
