📋 Description

• Collaborative Care Clinician I provides evidence-based interventions via the Collaborative Care
• Works under Clinical Excellence Manager and coordinates care with PCPs and other providers.
• Utilizes PHQ-9, GAD-7, and other tools to screen patients and monitor progress.

🎯 Requirements

• Master’s Degree in Social Work, Counseling, or related field from an accredited program
• Massachusetts licensed to practice (LCSW, LICSW or equivalent)
• 1-5 years of post-licensure experience as a licensed professional
• Fluent English and Spanish; strong medical interpretation/documentation skills
• Secure, high-speed internet for video conferencing and cloud-based systems
• Proficiency with EHRs; ability to quickly learn new technology
