Manager of Care Management, Health Homes (NY / Hybrid)

📋 Description

• Build and oversee a Health Home Care Management Program from the ground up.
• Maintain relationships with Health Home Leads; screen and enroll patients; onboarding, training
• Review billing and ensure NY State Health Home compliance regulations.
• Collaborate with VP of Operations and Team Lead to expand and improve the program.
• Develop workflows, policies, and procedures to operationalize care management.
• Build and manage a care management team, including hiring, training, and performance feedback.

🎯 Requirements

• Strong supervisory experience in Health Homes; 3+ years preferred.
• Master’s Degree in Social Work (MSW) or equivalent; LMSW/LSW licensure typical; license
• Demonstrated ability to travel in the field to accompany patients to appointments and provide
• Ability to navigate multiple data systems (e.g., Netsmart, Foothold, Salesforce) and conduct
• Experience with UAS-NY assessments and Health Home standards; ensure proper documentation and
• Excellent communication, empathy, and crisis-intervention skills; strong team leadership.
