Market Medical Director

📋 Description

• Lead clinical care across an assigned market
• Provide physician oversight and mentorship to NP/interdisciplinary teams
• Deliver primary care via in-home visits and telehealth
• Lead transitions of care for high-risk patients post-hospitalization
• Drive quality improvement and value-based care outcomes
• Collaborate with Operations to improve patient experience and market performance

🎯 Requirements

• MD or DO; board certified/eligible in Family/Internal Medicine or Primary Care
• Active, unrestricted license or ability to obtain licensure in the market
• Minimum 5 years of physician experience; leadership/mentoring experience preferred
• Background in value-based care, population health, or community-based healthcare preferred
• Strong communication and leadership skills; passion for serving older adults
• Comfortable working independently in field-based and virtual care settings
