📋 Description

• Senior clinical leader for Habitat Health PACE center
• Accountable for quality, safety, experience, and outcomes of care
• Dyad leadership with Executive Director for center performance
• Balance direct clinical care, leadership, team development, and fiscal stewardship

🎯 Requirements

• MD or DO with active, unrestricted license; DEA license
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty
• 3+ years of clinical experience caring for older adults or complex populations in PACE or
• Experience leading clinicians and interdisciplinary teams; strong coaching/mentoring skills
• Knowledge of CMS PACE regulations and quality improvement concepts
• Excellent communication and stakeholder management in a dyad leadership model
