Medical Director I

📋 Description

• Provide assessments, planning, primary care, and continuous medical coverage to participants
• Collaborate with community-based physicians and the IDT in discharge planning from acute or
• Actively participate in IDT meetings, communicating participant changes, collaborating on care
• Help maintain participant care in nursing homes, making regular visits as dictated by participant

🎯 Requirements

• Successful completion of a residency program accredited by the Accreditation Council for Graduate
• Master’s degree or doctorate degree
• Board certification – internal medicine, family practice, or emergency medicine; geriatrics
• Active State Physician License without restrictions, as well as state CDS certification and DEA
• Must be willing and able to participate in rotating night and weekend call
