📋 Description

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for client and family to navigate the community system.
• Collaborate with multidisciplinary BAYADA clinicians on the care plan.

🎯 Requirements

• Must be licensed by the VA Board of Social Work Examiners at LMSW or LISW level.
• Master's in Social Work with at least 2 years of community-based experience.
• Accreditation by CSWE (transcript or degree acceptable).
• Home care experience a plus.
• Strong reading/writing/communication in English.
• Ability to work independently and manage time effectively.
