📋 Description

• Make home visits to clients in designated geographic territories.
• Assist team in understanding social and emotional factors related to health problems.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resource referrals for practical and environmental needs.
• Advocate for clients and families to navigate the community system.
• Participate in the total plan of care with the multidisciplinary BAYADA team.

🎯 Requirements

• Licensed by the NJ Board of Social Work Examiners at LMSW or LISW level.
• MSW with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (transcript or degree).
• Home care experience is a plus.
• Strong English communication; ability to work independently; solid interpersonal skills.
• Proficient with computers; EMR experience preferred.
