Medical Social Worker, Home Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Assist our team in understanding social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals to address patient and family needs.
• Advocate for clients and families to navigate the community system.
• Participate in the development of the total plan of care with BAYADA clinicians.

🎯 Requirements

• Must be licensed by the SC Board of Social Work Examiners at LMSW or LISW level.
• Master’s in Social Work with at least 2 years of community-based experience.
• Accreditation by Council on Social Work Education (CSWE) demonstrated by transcript or degree.
• Home care experience a plus.
• Strong reading, writing and communication in English; ability to work independently and manage time.
• Strong interpersonal skills; solid computer skills; prior EMR experience preferred.
